**AIIMS Bhopal Performs First Sinus Venosus Atrial Septal Defect Closure Using Hybrid Covered Stent**
Bhopal (Madhya Pradesh): AIIMS Bhopal has successfully performed its first sinus venosus atrial septal defect (ASD) closure using a hybrid covered stent. The procedure employed a balloon-based flaring technique at the right atrium–superior vena cava (RA–SVC) junction, resulting in complete diversion of the right upper pulmonary vein (RUPV) flow unobstructed to the left atrium.
During the procedure, special care was taken due to the presence of an accessory right upper pulmonary vein, which required meticulous handling to prevent obstruction. Additionally, the caudal superior vena cava–right atrium (SVC–RA) junction was notably wide, necessitating deliberate stent flaring to ensure proper sealing and optimal blood flow.
The surgical team used a unique hybrid covered stent featuring a 15 mm uncovered portion at one end. To protect the accessory vein, a 5 French JR catheter was carefully passed through the stent struts at the junction between the covered and uncovered parts. The challenge of caudal edge flaring was innovatively managed using a 28 mm balloon mitral valvuloplasty (BMV) balloon. This approach ensured proper stent apposition and eliminated any residual shunting.
